Return the Facility Use Application (included in this packet) to the office no earlier than September 15, 2016.
Notifications of approval will be sent to you by email by September 30, 2016.
Final step, turn in a complete packet for your team to include: your Team Roster and ALL individual waivers. (Please do not have parents or students drop off waivers to the school. Coaches/Advisors must collect all individual waivers and turn in team rosters to the front office before the team can use the facility.)
